By completing this form, you acknowledge and agree to the following conditions of the membership:

• IF YOU CHOOSE TO PARTICIPATE IN ANY IN-SCHOOL OR ONLINE TRAINING SESSIONS/CLASSES: you understand and agree that the Tae E. Lee Ottawa Tae Kwon Do School Ltd. is not responsible for any loss or damage suffered by any person before, during or after any in-school or online training sessions should you choose to participate.  You agree to relieve and release the Tae E. Lee Ottawa Tae Kwon Do School Ltd., its agents, affiliates, contractees, employees, instructors, servants, and volunteers, of all liability for losses and damages of all and every description.   You also understand that for privacy and copyright reasons, recording of online sessions is not permitted.
• No refunds for any reason. I hereby agree that any payment that I have made or that I may make to the School will not be refunded under any circumstances, and that credit will not be given for classes not used for any reason or within validity period.
